Right, elephant in the room.
According to Korekiyo, his older sister suffered from a chronic illness that confined her to the hospital for most of her life and eventually killed her. Judging by how she introduced him to anthropology, pushed him to research as much as he could, and even customized his uniform for him, I assume this older sister was trying to live vicariously through her doting little brother.
This clearly was not a healthy relationship for either of them. Kiyo grew so attatched to his sister that they confused familial love with romantic love, and he had much trouble coping with her eventual demise. The near-death incident at a particular village did not help, which would explain why he now thinks her ghost possesses him (this is really a tulpa he created) and why he wants to murder 100 "admirable friends" so she wouldn't be lonely in the afterlife.
That being said, since Korekiyo is shown to prefer strictly physical relationships with others in canon I'm going with the headcanon that the incest thing is strictly romantic for the sake of everyone's sanity. Also, I will not have him murder your character unless we both want to thread that.
