Death. Korekiyo will have the wonderful pleasure of dying a slow, painful death from whatever coughing up blood disease his classmate Kaito was hiding. You tend to lose a lot of blood when your heart's trying to escape from your chest.
Phylactery. Kiyo's heart/soul/lifeforce will somehow attach itself to the locket he once wore around his neck. It tends to bleed a few minutes before he regenerates at it.
Disconnected limbs and extremities. Specifically, his head will detach from the rest of his body.
Manipulation of body. Kiyo's head can move around his torso like it did in life, but is also detachable. Please don't steal his head, you heathens.
Deathly body. Kiyo becomes a skeleton from the neck down. The remaining flesh is on his head has turned a deathly pale.
An affinity for decay. Decay magic! Kiyo can decay someone on touch, which can also shut down a human's senses, inflict terror or fear, bestow insomnia or nightmares, numb sensation or control of limbs, or kill the cells of their victim's body, prematurely aging them or causing their body to rot. Prolonged exposure is required for the nastier effects, and if he tries decaying a monster the effects will wear off in an hour.
The Hunger. Much like the hone onna, Kiyo feeds by draining the life force of humans, causing them to rot until they're nothing but bone. He has to have his locket on him when he feeds.
Regeneration. If Kiyo's body is destroyed, he will eventually regenerate near his locket. It takes a while, starting at 2 hours the first time and then adding on an hour every subsequent regeneration up to 24 hours. This counter resets every month. The only way to truly kill him is to destroy his locket, which won't be very difficult at all...
Optional
An "armored" helmet or mask. This is actually the same exact mask he always wears, except it's permanently stuck to his face. At least he doesn't need a special mask to eat anymore!
Absent body parts. Remember when I said he was skeletal from the neck down? Turns out, he doesn't actually have a neck anymore. Not even neck vertebrae. There is only cloth hanging from his mask.
The ability to cause inorganic decay. Just like he decays organic matter, he can cause objects to rot or rust. Very inconvenient for someone who likes ancient artifacts and such.
Quick escape. Once per day, Kiyo can demanifest himself and reappear near his locket. It's a one way trip, and he can't bring anyone or anything with him. Doesn't count towards his regeneration time decay.
